Wellington County OPP say five people are in hospital, including two children, following a crash north of Guelph on Monday morning.
Police said the head-on crash happened on Concession Road 6, south of Highway 89, which is just east of Mount Forest.
Two men were in one vehicle and a woman with two toddlers was in the other vehicle when the crash happened.
Const. Josh Cunningham said an elderly man suffered life-threatening injuries in the collision and was taken to a trauma centre.

The four others were taken to hospital with non-life-threatening injuries.
Police said it’s too early to say what caused the crash.
“One of the factors that we’re looking at is road conditions,” Cunningham said. “We see snow and a little bit of moisture on the road.”
He added that one vehicle had snow tires and the other had all-season tires.
